Scholarship Claim Instructions for Colleges and 2006 Scholarship Winners
regarding Papa John's Scholars, Leadership Scholars and Collegiate Scholars

THE AWARD AMOUNT IS $500 FOR FALL 2007 AND $500 (conditional) FOR SPRING 2008

We strictly follow IRS, Code Section 117 (b) (2) regarding scholarships. This scholarship award may only be used for tuition, books, supplies and equipment REQUIRED of every student in each particular class. Room and board and other expenses are considered incidental expenses by the IRS and are potentially taxable to the scholarship grantor and will not be funded by this scholarship. We pay all scholarship funds directly to the college, never to the student.

COLLEGE INSTRUCTIONS:
In the fall of 2006, two weeks after the student has started attending classes, mail us (not email or fax) verification that the student is enrolled. In the spring of 2008, if the student has a GPA of 3.0 on a 4.0 scale for previous fall semester, send us verification of enrollment and a transcript. If the award arrives after the student has paid their own money toward eligible items, the college may reimburse the student for that portion of the fees covered by that portion of the award. In a case where the student has been awarded other scholarships or grants that negate the benefit of this scholarship, we will re-award the funds to another deserving student. The college may not roll funds over for future semesters and is instructed to return unused portions of the scholarship to the Scholars Administrator.

STUDENT & PARENT INSTRUCTIONS & INFORMATION:
Be aware that your college will bill you long before we receive your enrollment verification and process your scholarship award, so plan ahead. Present a copy of this page to your college at the beginning of the fall 2006 semester. You must maintain a 3.0 out of 4.0 average as defined by your college in order to receive the second scholarship payment. All of your scholarship funds expire on June 1, 2008. The student is solely responsible for making sure that the college has processed this scholarship AND that funds have been received by their college prior to June 1, 2008. After that date, we irreversibly reassign unclaimed or unpaid scholarships to other students. The Scholar may attend any fully accredited college in the USA.

The Scholars Administrator will make every effort to assist the student, parents and the college in the processing of this award. Please do not hesitate to call me if there are glitches, concerns, questions or comments regarding the scholarship program.
